Doctor’s Note for Work or School Illnesses Icon

Key points

  • Our providers write doctor’s notes for work and school after a same day visit.
  • Notes can cover time off, a return date, and work restrictions.
  • You can get a note from an in person or telemedicine visit.
  • Walk in or book online. No appointment or referral needed.

Can urgent care give you a doctor’s note?

Yes. An urgent care doctors note is a written excuse from the provider who examined you. At Vitality, our providers write notes for work and school whenever your visit calls for one. Just ask during your visit, and you leave with it the same day.

What your doctor’s note includes

A note confirms you were seen and tells your employer or school what they need to know. It usually includes:

  • The date of your visit
  • The days you should stay home
  • The date you can return
  • Any work or activity restrictions
  • Your provider’s name, credentials, and our clinic contact information

Your diagnosis stays private. We leave it off the note unless you ask us to include it.

Doctor’s note for work

Many employers ask for a doctor’s excuse after a sick day. Our providers examine you, treat you, and document the time you need to recover. Ready to go back but not at full speed? We can write a return to work note with restrictions. Examples include light duty, lifting limits, or time away from certain tasks. Hurt on the job? See our page on work injuries.

Doctor’s note for school

Kids get sick, and schools often want a note before they return. We see children of all ages and write return to school notes after their visit. The CDC advises staying home until symptoms improve and fever has been gone for 24 hours without fever reducing medicine. Your child’s provider will give you a clear return date. Get a note from a telemedicine visit

Too sick to drive in? A telemedicine visit is $79 and can include a doctor’s note. Your provider decides if your symptoms can be assessed by video. Some employers require an in person visit, so check your policy first.

What we cannot provide

  • Backdated notes. A note covers the date we see you and the days after.
  • Notes without a visit. A provider has to examine you first.
  • FMLA or disability forms. We write doctor’s notes only. Your primary care doctor or specialist handles that paperwork.

What a visit costs

We accept most insurance plans. Without insurance, a basic visit is $119. See our full self pay price list.
