CDL FMCSA Medical Certification Changes 2025

Key Points
✅ No more paper DOT cards for CDL and CLP drivers
✅ Medical exam results will be sent digitally to your state licensing office (SDLA)
✅ Employers must use your CDLIS driver MVR to confirm medical certification
✅ CDL drivers must make sure their exam info matches their license exactly
✅ Medical variances (like exemptions or SPEs) will be linked to your exam and transmitted after approval
✅Paper cards will only be used for non-CDL drivers.
What You Need to Do
To avoid delays, follow these steps:
1. Schedule Your DOT Physical Early
Don’t wait until your card expires. Early testing gives time to fix errors.
2. Bring the Right Paperwork
If you have diabetes, sleep apnea, or another condition, bring notes from your doctor.
3. Match Your Info Exactly
Make sure your name, date of birth, license number, and state on the exam form match your driver’s license exactly.
4. Ask for a Copy
While not required anymore, you should still request a copy of your DOT card just in case.
5. Ask Questions
If you’re not sure how the new process works, we’re here to help — in English, Russian, Ukrainian, or Spanish.
What If There’s a Problem?
If you find out that your medical certification didn’t go through, take these steps:
Step 1: Contact the provider who performed your DOT exam and ask them to check for errors in the National Registry.
Step 2: Contact the Illinois SDLA (DMV) to confirm your status.
