DOT Exam FAQ
What is a DOT physical?
A DOT physical is a health exam for commercial drivers. It shows you are healthy enough to drive a truck, bus, or other large work vehicle safely. It is also called a CDL physical or DOT medical exam.
Can I walk in for a DOT physical?
Yes. Walk in at our Buffalo Grove or Lake Zurich clinic any time we are open. No appointment is needed, but you can book online to save your spot.
How long does a DOT physical take?
Most DOT physicals take 30 to 60 minutes. Bringing your medication list and health records helps your visit go faster.
How long is a DOT medical card good for?
A DOT medical card lasts up to 24 months. Your examiner may give you a shorter card, such as one year, if a health condition needs to be checked again sooner.
What should I bring to my DOT physical?
Bring your driver's license, a list of your medications, and your glasses, contacts, or hearing aids. If you have a health condition, bring recent records. Drivers who use insulin need Form MCSA-5870 filled out by their diabetes doctor.
What happens if I do not pass my DOT physical?
Your examiner will tell you why and what to do next. You may get a shorter card, need to bring more records, or come back after treatment. Your examiner makes the final decision.
Do I need a sleep study for a DOT physical?
No. Federal rules do not require a sleep study for every driver. If you already use a CPAP machine, bring a recent usage report from it.
Where do my DOT physical results go?
Your examiner sends your results to the FMCSA National Registry, a federal database, by midnight the next day. If you hold a CDL or learner's permit, FMCSA then sends your results to your state, which adds them to your driving record.
Do I still need to take my medical card to the Secretary of State?
Not in most cases. Since June 23, 2025, FMCSA sends CDL and learner's permit results to your state for you. Keep your paper copy in case your driving record has not updated yet.
When should I renew my DOT medical card?
Come in a few weeks before your card expires. If your medical card runs out, your state can take away your CDL privileges until you get a new one.
Pre-Employment Physical Exam
How much does a work physical cost?
A work physical at Vitality Urgent Care costs $95 at both the Buffalo Grove and Lake Zurich locations, with no facility fee. The price covers the exam and completing your employer's paperwork during the visit.
Does insurance cover a pre-employment physical?
Usually not. Employer-required exams are generally not a covered insurance benefit, so Vitality Urgent Care charges a flat self-pay price rather than billing a claim. Many employers pay the fee directly instead.
Can my employer be billed directly for my physical?
Yes. Employers with an account set up in advance are billed directly, so the employee pays nothing at the visit. Request an account using the form on this page, or call (224) 601-5001.
Do I need an appointment for a work physical?
No. Vitality Urgent Care takes walk-ins for work physicals seven days a week at both Buffalo Grove and Lake Zurich. You can also book a time online if you prefer a specific slot.
Can I get a work physical today?
Yes. Work physicals at Vitality Urgent Care are same-day, with no appointment required. Hours are Monday through Friday 8am to 8pm, and Saturday and Sunday 8am to 4pm.
Can I get a work physical on a weekend?
Yes. Both Vitality Urgent Care locations are open Saturday and Sunday from 8am to 4pm and accept walk-ins for work and pre-employment physicals.
How long does a pre-employment physical take?
Most pre-employment physicals at Vitality Urgent Care finish within the hour, paperwork included. Lab tests ordered at the same visit follow later. Immunity titer results come back in two to three days.
Will you fill out my employer's form?
Yes. Bring your employer's form to the visit and Vitality Urgent Care completes it before you leave, rather than mailing it afterward or asking you to come back a second time.
What if my employer did not give me a form?
Vitality Urgent Care has its own physical form. You leave with it completed, and your employer can keep it on file.
What should I bring to a work physical?
Bring your employer's paperwork, a photo ID, your glasses or contacts if you wear them, and a list of your current medications. Bring any immunization records if your employer asked about immunity.
How soon do I get my paperwork back?
The same visit. Your completed form goes home with you. Lab tests ordered alongside the physical follow later, with immunity titer results typically available in two to three days.

What is a DOT drug test?
A DOT drug test is a drug test regulated by the U.S. Department of Transportation (DOT). It is required for safety-sensitive employees in certain industries, including transportation and aviation, to ensure they are not under the influence of drugs or alcohol while on the job. Vitality Urgent Care offers same-day DOT and non-DOT drug testing.
Who needs a DOT drug test?
Employees who operate commercial motor vehicles (CMVs), such as trucks and buses, may be required to undergo DOT drug testing. This includes CDL and certain non-CDL drivers, as well as other safety-sensitive employees such as pilots and railroad workers.
What drugs are tested for in a DOT drug test?
DOT drug testing screens for five classes of drugs: marijuana, cocaine, amphetamines, opioids, and phencyclidine (PCP). Alcohol testing may also be required in certain situations.
How soon can I get my DOT drug test results?
DOT drug test results typically take 24 to 48 hours to process. Once available, results are reviewed by the Medical Review Officer (MRO) and reported to the designated employer representative (DER), as applicable. Additional testing or medical review may extend the turnaround time.
