SMS Messaging Program

Program Effective Date: March 19, 2026 · Page version 2.0 · Last reviewed July 17, 2026

Vitality Urgent Care offers an optional SMS text message program for patients who choose to opt in during registration, on paper, or by asking our front desk.

What we send. Messages about your care and account: appointment reminders, scheduling updates, visit follow-up instructions, laboratory notifications, prescription notifications, and billing notifications. Message frequency varies depending on your healthcare needs and interactions with the clinic.

Consent. SMS is voluntary and is not a condition of receiving medical care. SMS consent is part of our general Consent for Treatment form, presented to you during routine registration — there is no separate form. By providing your mobile number and signing the consent form, you agree to receive the messages described above. If you prefer not to receive text messages, simply tell our staff at registration and no messages will be sent — and you can stop messages at any time by replying STOP. We keep a record of your signed consent and of any opt-out.

Opt out anytime. Reply STOP to any message. You will receive one final message confirming you have been unsubscribed, and no further messages will be sent unless you opt in again.

Help. Reply HELP to any message, email info@vitalityurgentcare.com, or call 224.601.5001.

Rates and carriers. Message and data rates may apply depending on your mobile carrier and plan. Messaging is supported by most major U.S. carriers but is not guaranteed on all carriers or devices; carriers are not liable for delayed or undelivered messages. Do not rely on SMS for urgent communication — if this is a medical emergency, call 911.

Privacy. Your mobile number and opt-in consent are used only for the communications described above and are never shared with third parties or affiliates for marketing or promotional purposes. Sharing with subcontractors that operate our messaging service is permitted only as necessary to deliver messages. Standard SMS is not a fully secure channel; limited health-related information (such as appointment or result-availability notices) may appear in message content. See our Website Privacy Policy and Notice of Privacy Practices.
