Patient Portal (Experity) — If you use our patient portal powered by Experity, any health information you access or submit through the portal is protected health information governed by HIPAA and our Notice of Privacy Practices, not solely this Website Privacy Policy.

Non-Personal Information — We collect non-personal data such as browser type, IP address, and pages visited to improve our website and services. This data does not identify you individually.

For website-related personal data (non-PHI), you may contact us to request access, correction, or deletion, subject to applicable law.

Data Security

We implement administrative, technical, and physical safeguards designed to protect your personal and health information from unauthorized access, alteration, disclosure, or destruction, in compliance with HIPAA Security Rule requirements (45 CFR Part 164, Subpart C). No internet-based transmission is 100% secure; we cannot guarantee absolute security.

SMS Messaging Privacy

If you opt in to SMS messages, your mobile phone number and opt-in consent information are used only for healthcare-related communications and are never shared with third parties or affiliates for marketing or promotional purposes. Sharing with subcontractors that operate our messaging service is permitted only as necessary to deliver messages. You may opt out at any time by replying STOP. Illinois-Specific Disclosures

Illinois residents may have additional rights under the Illinois Personal Information Protection Act (815 ILCS 530) and the Illinois Health Information Exchange and Technology Act. Cookies

Our website uses Google Tag Manager to manage analytics tags that help us understand how visitors use the site.
