What If Something Is Detected?

- Order on-site labs, bloodwork, or X-rays
- Provide prescriptions or follow-up referrals
- Coordinate specialist care if necessary
On-Site Services Available
Need more than a physical? We provide in-house diagnostic services if further evaluation is needed:
- Digital X-rays: For injuries, joint pain, or range-of-motion concerns.
- EKG (Electrocardiogram): If needed for cardiac screening or history of chest pain or fainting.
- Rapid testing: Strep, flu, COVID-19, RSV, mono, and more — with results during the visit.
- Lab work: Basic blood panels when further evaluation is warranted.
- Vision screening: Quick checks to meet school form requirements.
- Urinalysis: Per request for certain programs or forms.

When is a school or sport exam required?
School physicals are typically required before Kindergarten, 6th grade, and 9th grade in Illinois.
Sports physicals are required every year before a student can participate in school sports or athletic activities.

What forms do I need?

Illinois Certificate of Child Health Examination (for school physicals). Also accepted for sports participation in most schools—no need to fill out both forms if this is completed

IHSA Pre-Participation Physical Form (for sports physicals). Required only if your child does not need a school physical that year and is playing sports.
Our provider will complete and sign the appropriate form during your visit. Forms can also be provided on-site if you don’t bring one with you.
